A review at Stockport identified similar concerns, including failures raised by a whistleblower and multiple patient safety investigations.
Cases like these often mean that a child failed newborn hearing screening tests, but was not referred promptly for further assessment. It can also mean that testing protocols were inadequate or not followed, national guidelines on childhood hearing were misunderstood or ignored, or appointments for further investigation – such as testing under anaesthesia or ENT referral – were delayed without good reason.
